Pharma-Care, Inc: We are currently seeking a Clinical Pharmacist to service group homes throughout Delaware. This is a per diem position that involves visiting group homes in locations such as Millsboro, Georgetown, Seaford, Dover, Frankford, Bear, Milford, Smyrna, Newark, Dagsboro, DE, plus other locations. Must have DE Board of Pharmacy license.

Responsibilities – A day in the life of a Clinical Pharmacist consultant:

  • Travel to group homes in your region.
  • Evaluate the appropriateness and effectiveness of medication regimens.
  • Areas of focus will include antimicrobial therapy, pain management, use of psychoactive medications.
  • Collaborate with physicians and other health care providers in managing medication therapy to ensure patient needs are met.
  • Apply specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions.
  • Active role in the development and implementation of protocols, antimicrobial stewardship initiatives and appropriate drug utilization.
