Service Coordinator Position

Crescent Park Senior Housing, a HUD-subsidized rental affordable independent senior housing building with 73 units located in Spotswood, NJ is seeking a full-time service coordinator.

Requirements:

Bachelor of Social Work or degree in Gerontology, Psychology or Counseling preferable; or high school diploma or general education degree (GED) and four to six years related experience and/or training in the field of aging and senior issues.

Must have a working knowledge of supportive services and other resources available to seniors; and experience with eligibility and procedures of Federal and NJ entitlement programs.   Able to work independently and as a team member.

Duties Include:

  • Facilitates the maximum level of county, state, and federal benefits for tenants and ensures all documentation is processed in a timely manner.
  • Performs service assessments on all new tenants to determine whether any additional services are required in order for them to live in the facility.
  • Advocates for all tenants via the building mission statement.
  • Arranges for tenant transportation to applicable medical facilities as needed.
  • Prepares and forwards all required HUD reports to the main office for official submission to HUD.
  • Stays abreast of relevant aging issues by utilizing one or more of the following: seminars, conferences, advocacy groups, internet research, and literature.
  • Coordinates periodic educational programs related to tenants’ physical, mental, and spiritual well-being.
  • Schedule and organize health clinics (flu, vaccine, etc.).
  • Community outreach.
  • Ensures all tenant recordkeeping is organized, legible, and updated as needed with any applicable changes.
  • Performs other related duties as assigned.

Employer Provided Specialized Training:

  • 36 hours of classroom/seminar training covering nine specific subject areas must be completed during the first year of employment.
  • 12 hours of continuing education is required for each successive year of employment.
