The Atrium at Navesink Harbor in Red Bank, NJ is seeking a Social Worker/Admission Coordinator for our forty-three bed Healthcare Community.
There are two primary purposes of this position: One is to maintain facility occupancy by coordinating admissions to the Healthcare Center (HCC) according to Springpoint Senior Living (SSL) objectives. The second is to perform the services of a social worker in accordance with New Jersey State regulatory requirements and within Springpoint policies and procedures.
Additional Responsibilities include:
Establish and maintain a working relationship with the facilities primary hospital case managers/discharge planners, social workers and related community stakeholders and advise/educate about health care services available at SSL community.
Actively follow up on all referrals from both internal and external sources.
Meet with new residents discuss availability of Advance Directive, answer questions, provide Health Care Center Resident Handbook and Resident Bill of Rights.
Obtain signatures for Bill of Rights, Ombudsman Disclosure Form, Chaplain Authorization Form, Laundry Form.
Complete designated portion of comprehensive Medicare MDS by fifth day and in accordance with required MDS practices for a social worker.
Attend and complete required documentation in preparation of Interdisciplinary care plan conferences, including in in person or remote family participation upon invitation.
Education/Experience: LSW and/or MSW Required. Experience in LTC Admissions. Experience in LTC Social Work. Working knowledge of state and federal health insurance benefit programs and plans along with Medicare Advantage/HMO programs and plans. Possess good organizational and communication skills and ability handle multiple projects in a fast-paced environment. Must have knowledge of Microsoft Word, PowerPoint, and Excel.
This position is temporary Full Time and may become a permanent employment opportunity.
Temporary position duration is October 31 2022 through September 2023.
Training will be provided from 10/31/2022 through January 31, 2023.
Sign on Bonus $2000.00. Hourly rate:$30/hour.
