SUMMARY:Identifying Problems Related to Medication Adherence in Patients Receiving Home Healthcare
DESCRIPTION:Patients may be non-adherent to the their medication regimens for a variety of reasons. Asking the right questions may help clinicians with any level of experience identify the presence of non-adherence and the potential reason for non-adherence. Having identified the presence of problem\, the information can be shared with other providers to bring the problem to resolution.  \nParticipants will  gain knowledge of a tool to identify problems related to medication adherence by participating in this program.  \nPresenter Information  \nJeffrey Clark\, Clinical Assistant Professor\, Washington State University\, and Consultant Pharmacist  \nDr. Jeffrey Clark completed his PharmD in 2009 at Washington State University College of Pharmacy and Pharmaceutical Sciences. After graduation he completed a 1 year residency in geriatrics with WSU and Providence Visiting Nurses Association (PVNA). After working as a full-time consultant pharmacist in skilled nursing facilities\, he returned to WSU & PVNA in 2013 where he is now a clinical assistant professor and a consultant pharmacist. At PVNA he runs an Advanced Pharmacy Advanced Pharmacy Practice Experience for Doctor of Pharmacy candidates focused on preventing hospitalizations in patients with high risk conditions (e.g. heart failure\, COPD). He has a board certification in geriatrics and is passionate about geriatric medicine and maintaining independence at home. SUMMARY:Home Care Becomes Conversation Ready: Using Tools from the Conversation Project to Increase Advance Care Planning Conversations
DESCRIPTION:Patty Webster\, MPH\, Institute for Healthcare Improvement\, and Kate Lally\, MD\, FAAHPM\, Dana Farber Cancer Institute & Institute for Healthcare Improvement\, will share a variety of tools available from the Institute for Healthcare Improvement and The Conversation Project to help increase conversations around end of life care planning in your home-based patient population. They will also share stories of how other home care and hospice agencies have used these tools to increase documentation of advance care planning and lessons learned on promoting these services to your community.  \nParticipants will gain insight on strategies and tools for more effective ACP conversations and ways to get involved with The Conversation Project by participating in this program.  \n  \n Patty Webster serves as Faculty and Improvement Advisor (IA) for Community Engagement for The Conversation Project\, coaching community partners\, leading monthly community calls\, and supporting the team’s knowledge management\, measurement\, learning\, evaluation\, and dissemination efforts. Since 2007\, she has served as IHI Faculty and IA supporting community-based and health system improvement efforts in the US and Africa. Her roles have included facilitating cross-organizational and multi-country learning networks/communities of practice\, designing and leading quality improvement collaboratives\, and contributing to IHI’s initial strategy on person- and family-centered care.  \nKate Lally\, MD\, FAAHPM\, serves as a Senior Physician in Psychosocial Oncology and Palliative Care at Dana Farber Cancer Institute and an Instructor at Harvard Medical School.
