SUMMARY:Navigating Your Path to Success Under the SNF Patient Driven Payment Model
DESCRIPTION:OVERVIEW: \nJust 95 days from the date of this seminar\, the Patient Driven Payment Model will take effect for skilled nursing facilities. Is your nursing home ready? Are hospitals ready? This seminar will feature Health Dimensions Group’s team of experts as captains of the journey to success under PDPM. Starting with the most current policy clarifications from CMS and context under Medicare reform\, this program will engage attendees in activities that gauge readiness and steer them towards areas to focus on as they are making final preparations for implementation this summer. Our focus will be on rehabilitation\, coding and documentation\, and operational excellence. The program will utilize case studies and an interactive approach in order to ensure that key takeaway messages are reinforced. \nOBJECTIVES: \n  \n• To learn the latest PDPM policy and the larger context for Medicare reform \n• To identify strategies for operational and programmatic success \n• To understand gaps in SNF readiness for PDPM \n  \n  \nTARGET AUDIENCE: \nSkilled nursing facility administrators\, department directors\, MDS and coding teams\, finance teams and acute care hospital case managers and finance teams \n  \nClick HERE to Register

SUMMARY:Achieving Success for Your Skilled Nursing Facility: Mastering Coding of Section GG & ICD-10 Diagnosis Coding for PDPM
DESCRIPTION:OVERVIEW: \nSkilled Nursing Facilities are preparing to transition from the current RUGs IV PPS reimbursement model to the Patient Driven Payment Model (PDPM) effective October 1\, 2019. There are two primary coding changes that will impact reimbursement under PDPM. The first one is the change from the use of the MDS Section G to using Section GG for the functional components for the PT\, OT and nursing Case Mix Indexes (CMI). The second big change is using diagnosis coding as the primary reason for the SNF stay to support the patient characteristics for PT\, OT\, ST\, Nursing and the Non-Therapy Ancillary (NTA) CMI and reimbursement. This seminar will focus on both the know-how to identify coding inaccuracies\, CMS’ intent regarding correct diagnosis and Section GG coding and improved documentation to support both diagnosis coding and Section GG coding. Expert faculty will lead participants through real examples to develop coding mastery. \nAt the end of this presentation the attendee will: \n• Understand the transition from Section G to Section GG coding for the functional component and reimbursement starting October 1\, 2019 \n• Become aware of current coding inaccuracies of the submitted Section GG on the 5-day MDS \n• Demonstrate an understanding of the intent and guidelines of Section GG as outlined by CMS \n• Be able to define and explain each component of Section GG for PT\, OT and nursing \n• Apply the coding scale accurately to capture the usual performance utilizing real SNF patient examples \n  \n• Demonstrate documentation supporting coding of Section GG for reimbursement purposes \n• Understand the PDPM requirements for identifying the reason for the SNF stay \n\nReview risks associated with incorrect diagnosis coding\, including the impact of RTP codes\nLearn to utilize the CMS tools for correct diagnosis coding under PDPM\nPractice coding scenarios for SNF admission examples using coding manuals/other tools \n\n  \n  \nTARGET AUDIENCE: \nSMF MDS coordinators and RNACs\, clinicians\, coders\, corporate\, compliance\, officers\, rehabilitation therapists and others involved in the SNF MDS process and reimbursement \n  \nClick HERE to register

SUMMARY:Unique Aspects of Advance Care Planning with Assisted Living Residents & Families
DESCRIPTION:New Jersey’s annual assisted living resident profile survey has demonstrated that residents are aging in place and new residents are older and have more complex healthcare needs when they come to live in assisted living communities. As these trends continue\, communities will continue to grapple with advance care planning and the challenging conversations surrounding the clinical capabilities of assisted living communities as well as the expectations of families\, residents and staff. This half day program will help communities’ administrators\, nurses\, social workers and others involved in the advance care planning understand the issues from different perspectives and provide practical strategies for more effective advance care planning. \nRegister here \n
