A-2: Leveraging Technology for Collaboration Across the Continuum to Improve Patient Outcomes

Session Description:

RWJBarnabas Health will discuss how the implementation of Nursing Advantage by their SNF Performance Network Partner Parker at Somerset improved the quality of notes shared through Point Click Care PAC Network Management. This session will also review the importance of leveraging technology to decrease staff burn out and improve communication leading to positive patient outcomes. Finally, this session will explore benefits of implementation of technology such as Nursing Advantage to standardize their documentation process for their front line staff.

Danielle Salazar

Director – Post Acute Network, RWJBarnabas Health

Danielle Salazar is currently the Director of the Post-Acute Network at RWJBarnabas Health. In her role, she collaborates with the post-acute partners to identify opportunities that would increase communication and quality care across the continuum. She has worked in skilled nursing facilities, acute care hospitals, and value-based programs. Danielle is well known among many post-acute care providers across New Jersey for her coordination efforts at Signify Health, formerly Remedy Partners. She has interfaced on the post-acute side working closely with skilled nursing facilities on key performance indicators in order to improve patient outcomes. She completed her Masters in Science in Nursing Leadership at Grand Canyon University and is a member of The American Nurses Association, The American Case Management Association and The Association of Professionals in Infection Control and Epidemiology.
