Home Care Becomes Conversation Ready: Using Tools from the Conversation Project to Increase Advance Care Planning Conversations

August 21, 2019 @ 12:00 pm - 1:00 pm

Patty Webster, MPH, Institute for Healthcare Improvement, and Kate Lally, MD, FAAHPM, Dana Farber Cancer Institute & Institute for Healthcare Improvement, will share a variety of tools available from the Institute for Healthcare Improvement and The Conversation Project to help increase conversations around end of life care planning in your home-based patient population. They will also share stories of how other home care and hospice agencies have used these tools to increase documentation of advance care planning and lessons learned on promoting these services to your community.

Participants will gain insight on strategies and tools for more effective ACP conversations and ways to get involved with The Conversation Project by participating in this program.
