A-4: Pursuing Equity for Older Adults Facing Social Risk Factors

Session Description:

Three aging services organizations, Jewish Home Family, Springpoint Senior Living and Bergen New Bridge Medical Center in New Jersey will share their journey as part of a regional collaborative called Leaders in Equity and Diversity (LEAD) in 2022. LEAD has been convened by Meritus Health and AARP and includes healthcare organizations of many types from six other states. With a focus on addressing inequities in access to services, particularly preventive and primary care, the three organizations embarked on initiatives designed to reach into their communities, employ new strategies and techniques, and move the needle on identification and action related to health related social risk factors, also called social determinants of health. Participants in this session will learn the approaches and strategies that have been successful, the lessons learned and pitfalls encountered, and the exciting plans for enhancing and expanding these efforts.

Theresa Edelstein, Regulatory Consultant, LeadingAge NJDE & NJHA

Theresa Edelstein has been in the aging services field for more than 25 years, including as a licensed nursing home administrator, planner and advocate.

Carol Silver-Elliot, Chief Executive Officer, Jewish Home Family

Carol Silver Elliott is the President and CEO of the Jewish Home Family, located in northern New Jersey. Serving older adults for more than a century, the Jewish Home continuum of services includes the Jewish Home at Rockleigh, offering both long term care and short term rehabilitation; Jewish Home Assisted Living; Jewish Home at Home, providing home health care services; Jewish Home Foundation as well as a nationally recognized program for the care of individuals with Parkinson’s disease. Prior to joining the Jewish Home Family in 2014, Silver Elliott served as the President and CEO of Cedar Village Retirement Community in Cincinnati, Ohio.

Saswati Chakraborty, Case Manager, Springpoint Senior Living

Saswati Chakraborty brings over 25 years of healthcare experience, specializing in marketing, business development, and admissions. Currently serving as Clinical Case Manager at Springpoint’s Monroe Village and Village Point Rehabilitation & Healthcare, she coordinates clinical case management, acting as a liaison between various healthcare providers and services. With expertise in assessment, treatment planning, and advocacy, she holds specialized training in ACOs, bundled payments, and managed care. Chakraborty is certified in Alzheimer’s Dementia and has completed extensive training, including a certification course from the University of Tasmania’s Wicking Dementia Center. She is also a PAC Dementia Trained Coach and a certified facilitator of caregiver support groups for Alzheimer’s Dementia. With a Ph.D. in Clinical Psychology and dual master’s degrees in business administration and clinical psychology, she is a certified social worker and dementia practitioner, actively contributing as a member of the Geriatric Advisory Council.

Donnalee Corrieri, Chief Communications & Marketing Officer, Bergen New Bridge Healthcare

Donnalee Corrieri Chief Marketing and Communications Officer Donnalee Corrieri has more than 25 years of experience in the advertising, public relations and marketing field. She is currently the Chief Marketing and Communications Officer at New Jersey’s largest hospital, Bergen New Bridge Medical Center where her job responsibilities include employee relations, public/media relations (hospital spokesperson), marketing, advertising, crisis communications, and most recently, diversity, equity, and inclusion. She was selected to give the 2019 Bergen County Pride Ceremony Keynote Speech and is a member of the LGBTQ+ community. She is a board member for New Jersey Friendship House and the YWCA Northern New Jersey and sits on the Paramus Stigma Free Initiative committee.
