D2: How can digital transformation put patients at the center?

This continuing education program is sponsored/conducted by LeadingAge New Jersey, dba LeadingAge New Jersey & Delaware, and is in compliance with N.J.A.C. 8.34-7.3 to provide licensed nursing home administrators (LNHA) and certified assisted living administrator (CALA) education credits accepted by the Nursing Home Administrators Licensing Board. This education activity may only be used for continuing education credit and not to meet academic college credits.

In support of improving patient care, this activity has been planned and implemented by AXIS Medical Education and LeadingAge NJ/DE.  AXIS Medical Education is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

This panel discussion will explore the integration of technology in long-term care, highlighting both opportunities and challenges. Facilitated by Jennifer D’Angelo, the session will feature insights from a Chief Information Officer (CIO), a long-term care resident, and a patient experience expert. The CIO will discuss how digital transformation, data analytics, and telehealth solutions can streamline operations, reduce costs, and improve patient-centered care. A long-term care resident will share firsthand experiences, emphasizing the impact of user-friendly technology on daily life, independence, and well-being. The patient experience expert will provide a broader perspective on how a patient-centered approach to technology adoption enhances healthcare experiences and outcomes. The discussion will also address barriers to adoption, such as digital literacy and access, and propose strategies to ensure equitable technology integration in senior care. Attendees will gain valuable insights into leveraging technology to improve care delivery and patient engagement.
