G4: Steppingstones in the Right Direction: Building a Proactive Interdisciplinary Wellness Partnership in Older Adult Housing

This continuing education program offers continuing education credit for Affordable Senior Housing Managers and Resident Service Coordinators to be submitted to HUD. Education covers federal/state senior affordable housing policy, ethical concerns, fair housing compliance, HUD Handbook training, financing green retrofits, supportive services, and solutions for aging properties and residents.
In support of improving patient care, this activity has been planned and implemented by AXIS Medical Education and LeadingAge NJ/DE. AXIS Medical Education is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
Meeting the evolving wellness needs of aging residents presents an ongoing challenge for housing providers, especially as older adults strive to maintain independence in communities they value. This session examines the partnership between Northgate II, a 302-unit affordable housing site in Camden, NJ, and Enhance Therapies, a national provider of onsite physical and occupational therapy. By focusing on a resident-centered approach to wellness, the session explores the strengths each partner brings to the table and highlights practical strategies for aligning goals. Attendees will also learn about the opportunities and challenges of forming collaborative partnerships that address wellness needs effectively.

Speaker Information
Vice-President of Post-Acute Care Services, Enhance Therapies
John Rocha is the Vice-President of Post-Acute Care Services for Enhance Therapies. John is a Doctor of Physical Therapy with 20 years of experience practicing in a multitude of care settings along the continuum of care, He began practicing in the outpatient setting while simultaneously starting a homecare practice. His career path then steered him into skilled nursing care as a director of rehabilitation for 10 plus years. Dr. Rocha’s perspective on patient care, effective management, and interpersonal skills lead to his advancement from the clinic to Vice President. He is responsible for forging strategic partnerships, developing a referral platform that allows for a collaborative interdisciplinary approach to post-acute care and expanding the coordination of post-acute patient care in the home setting. Dr. Rocha earned his Bachelor of Science in Biology from The College of New Jersey. He continued his education at New York University, where he earned his Doctor of Physical Therapy.
Department Head, Social Services, Fair Share Housing Development
Since 2022, Marilyn Mock has served as Department Head for Social Services at Fair Share Housing Development (FSHD) based out of Fair Share Northgate II in Camden, NJ, which provides housing to 650 older adults, individuals with disabilities and families with children. In her current position, Marilyn is responsible for overseeing social service provision at all FSHD properties and developing and sustaining partnerships with other non-profit organizations, healthcare institutions, academic institutions, government entities, foundations and other organizations that support the social service work of FSHD. Marilyn’s previous positions at Fair Share include Policy Director for Wellness Services, Northgate II Director of Social Services. Marilyn has over 30 years of experience working with older adults and individuals with disabilities.
