Provider Member Application Form
LeadingAge New Jersey & Delaware offers a graduated dues program that allows new provider members to join at 50% of the current dues rate for the first year, and full dues rate effective the second year of membership.
MEMBERSHIP DUES
LeadingAge New Jersey & Delaware Member Dues are a millage-based (program service revenue) dues structure. This single dues methodology applies to all members across the continuum of housing, care, and services. Member dues are calculated based on self-reported program revenue.
LeadingAge New Jersey & Delaware offers a graduated dues program that allows new facility members to join at 50% of the current dues rate for the first year, and full dues rate effective the second year of membership.
Members are automatically enrolled as members of LeadingAge. LeadingAge’s calculation of dues is a tier-structured, millage system based on program service revenue (Sources: Line 2-IRS Form 990, Facility Cost Medicaid Report, or annual financial audit information), and excludes realized gains/losses and charitable contributions.
